My own thoughts:
First of all, i don't think it's hard to understand until page 7. Then it becomes harder and harder... So that's why this was my first cut...
I noticed that i was wondering what kind of relationship those two people have and how they feel for each other. There is the english man who on the one hand really depends on the nurse, because she takes care for him, washes him, gives him food - all the things he isn't able to do for himself anymore...
I think they are very close, maybe there also is some kind of love ...
